THE CEILING-MOUNTED EXIT SIGNS AT CORRIDOR INTERSECTIONS WERE SHOWN TO BE LOCATED SUCH THAT THEY PARTIALLY BLOCKED THE LINE OF SIGHT TO THE BACKLIT WAYFINDING SIGNS, AND WERE RELOCATED SUCH THAT VISUAL INTERFERENCE WITH THE WAYFINDING SIGNS WAS MINIMIZED. THE IT VOICE RISER CABLES ARE SHOWN TO BE TERMINATED ON RACK-MOUNTED "110 BLOCKS", BUT DURING THE COORDINATION PROCESS WITH VAMC IRM STAFF, THE 110 BLOCKS WERE CHANGED TO "66 BLOCKS" FOR THESE TERMINATIONS. THE TRANE HPC-1 UNIT'S OVER CURRENT PROTECTION WAS FOUND TO BE FUSED AT TOO HIGH AN AMPERAGE LEVEL AND WAS REQUIRED TO BE MODIFIED BY THE ADDITION OF FUSE REDUCERS INSIDE THE FUSED DISCONNECT SWITCH PLUS THE INSTALLATION OF LOWER-AMPACITY FUSES SO AS TO BE WITHIN MANUFACTURER'S RECOMMENDATIONS. TWO ISOLATION ROOM ALARM PANELS IN ROOM'S 1B-165 AND 1B-123 THAT WERE NOT PROVIDED WITH ELECTRIC POWER ON THE ORIGINAL DRAWINGS WERE REQUIRED TO BE POWERED BY EXTENDING TWO ELECTRIC CIRCUITS FROM NEARBY PANELS THAT WERE ALREADY POWERED. THE ELEVATOR ROOM 6A-114 ON LEVEL 6 DID NOT SHOW FOR A ILLUMINATED EXIT SIGN ON THE DRAWINGS AND ONE WAS REQUIRED TO BE ADDED TO COMPLY WITH LIFE SAFETY CODES. THE REVERSE OSMOSIS WATER TREATMENT SYSTEM WAS SHOWN TO REQUIRE A SINGLE-PHASE CIRCUIT FOR ELECTRICAL POWER ON THE ORIGINAL PROJECT DRAWINGS, BUT THE EQUIPMENT REQUIRED THE POWER TO BE INCREASED TO THREE-PHASE POWER AND REQUIRED THAT A LOCAL DISCONNECTING MEANS BE PROVIDED.

THE AIR FLOWS AND COOLING REQUIRED IN THE CT WAS NOT ADEQUATE IN THE ORIGINAL DESIGN. ASI #177 REVISES THE HVAC IN THE CT ROOM.

    Veterans Affairs Department
OFFICE OF CONSTRUCTION & FACILITIES MANAGEMENT
SEVERAL AREAS OF THE CLINIC HAVE BEEN IDENTIFIED THAT ARE NOT DESIGNED OR INSTALLED PER VA HVAC DESIGN MANUALS. THE A/E HAS PROVIDED DESIGN RECOMMENDATIONS TO ACHIEVE COMPLIANCE. THE VAMC HAS REQUESTED THAT THESE CHANGES AND MODIFICATIONS BE IMPLEMENTED ON AN EXPEDITED BASIS TO SUPPORT CLINIC OPENING AT YEAR-END. THE GENERAL CONTRACTOR HAS AGREED TO ACCEPT THE CONTRACT CHANGE ORDER TO PERFORM THE WORK.
  • $95,424 - Monday the 21st of October 2013
    Veterans Affairs Department
OFFICE OF CONSTRUCTION & FACILITIES MANAGEMENT
DURING A REVIEW OF THE GOVERNMENT PURCHASED CONTRACTOR INSTALLED BIOLOGICAL CABINET IN ROOM 1B-107A, IT WAS NOTICED THE A/E DID NOT DESIGN AN EXHAUST SYSTEM FOR THE BIO CABINET THAT WAS TO BE INSTALLED IN THE LAB. THE DESIGNED AND INSTALLED SYSTEM WAS NOT SIZED PROPERLY AND A NEW SYSTEM HAD TO BE DESIGNED AND INSTALLED.
  • $9,220 - Monday the 21st of October 2013
    Veterans Affairs Department
OFFICE OF CONSTRUCTION & FACILITIES MANAGEMENT
CONTRACTOR RECEIVED COORDINATION INSTRUCTIONS FROM THE VA FROM A/E FOR ELECTRICAL WORK, SOME OF WHICH REPRESENTED AN INCREASE IN SCOPE OF WORK ABOVE WHAT WAS DEFINED IN THE CONTRACT DRAWINGS. THIS WORK INCLUDED ADDING A CORD AND CHANGING AN OUTLET, ADDING FEED TO PLUMBING FIXTURE, ADDING FEED TO WATER COOLER, ADDING DISCONNECT FOR CH-4, AND PROVIDING SURGE SUPPRESSION FOR WATER TANK PANEL.
  • $9,005 - Monday the 21st of October 2013
    Veterans Affairs Department
OFFICE OF CONSTRUCTION & FACILITIES MANAGEMENT
THE ORIGINALLY-PERMITTED AND DRILLED WELL DID NOT PROVIDE WATER AT AN ADEQUATE RATE TO SATISFY THE IRRIGATION NEEDS OF THE NEW FACILITY. THE WELL WAS RE-DRILLED TO A DEEPER LEVEL WITH A GREATER CAPACITY THAT WOULD MEET THE NEEDS. THE RE-DRILLED WELL UNEXPECTEDLY WAS AN 'ARTESIAN WELL' WITH NATURAL PRESSURIZATION THAT FLOWED WATER EVEN WHEN THE WELL PUMP WAS NOT RUNNING. THE ARTESIAN PRESSURE CAUSED WATER TO FLOW INTO THE IRRIGATION SYSTEM WATER HOLDING TANK AND TO EVENTUALLY OVERFLOW ONCE THE TANK WAS FULL. THE ADDITION OF AN ELECTRICALLY-ACTUATED VALVE WAS REQUIRED TO BE ADDED TO PREVENT THE WATER TANK FROM BEING OVER-FILLED AND FROM OVERFLOWING.
  • $8,845 - Monday the 21st of October 2013
    Veterans Affairs Department

AFTER REVIEW OF THE SECURITY DRAWINGS WITH THE VA POLICE, THE VA POLICE REQUESTED ADDITIONAL CAMERA, RELOCATED CAMERA AND ADDITIONAL CARD READERS TO PROVIDE BETTER OVERALL SECURITY FOR THE FACILITY.
