CAPITAL IMPROVEMENT&MAINTENANCE CLEVELAND NF WATER WELL IMPROVEMENTS MODIFICATION NO. 4 IS EFFECTIVE NOVEMBER 14, 2011. IMPORTANT NOTE: THIS MODIFICATION HAS NOT BEEN FUNDED WITH AMERICAN RECOVERY AND REINVESTMENT ACT FUNDS AND THEREFORE IS NOT SUBJECT TO FAR CLAUSE 52.204-11. PLEASE DO NOT INCLUDE THESE FUNDS AND ACTIVITIES IN YOUR QUARTERLY REPORT SUBMITTED TO WWW.FEDERALREPORTING.GOV. THE PURPOSE OF THIS MODIFICATION IS TO INCREASE THE TOTAL VALUE OF THE CONTRACT TO ALLOW FOR THE STABILIZATION AND IMPROVEMENT OF THE WATER WELL. STABILIZING THE FRACTURES INCLUDE REAMING OUT THE HOLE TO A LARGER DIAMETER AND SETTING IN A WELL LINER CASING THE ENTIRE DEPTH OF THE WELL AND INSERTING GRAVEL BETWEEN THE CASING AND THE BOREHOLE TO HOLD BACK THE CRUMBLING WALL. THE COSTS ASSOCIATED WITH THE ADDITIONAL WORK IS $66,601.33. THE OVERALL VALUE OF THE CONTRACT IS HEREBY INCREASED FROM $119,352.00 TO $185,953.33, A NET INCREASE OF $66,601.33. ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED.
